Before the incident
Assisting in rig up of power tongs on rig floor
What happened
[REDACTED] rig up process the TRS crew picked up the lift cylinder from out the of the power tong 's cam and set it on the door of the tongs. The air hoist was connected to the top part of the lift cylinder for lifting. The shackle connecting the bottom part of the lift cylinder to the power tong frame was in a bind and unable to be removed. The hoist operator was signaled to give some slack to the air hoist line to free up the shackle for removal. This allowed the crew to maneuver the cylinder and the shackle bolt to come free thus allowing lift cylinder to be removed from the power tong frame. The excess slack length on the air hoist line then allowed for the lift cylinder to swing down. The IP had their left hand on the lift cylinder and their right hand on the Power Tong when the lift cylinder came down and pinched their right-hand pinky finger in the IP.
