What happened
While EE was rigging a beam they were unaware that one chain was around the beam and the other was around a plate on the beam. This caused the chains to not pick up at the same time. The chain around the plate caught first and flipped the beam toward the employee while still hooked up striking their left index finger causing a laceration.
