employee mentioned to me on [REDACTED] that she had an ache in her shoulder and was unsure of the cause. She agreed to see impact ergonomics to see if PT would help. I told her to update me if anything gets worse. Impact ergonomics did not seem to think this was a major issue and I never received news from her of it getting worse. An incident report was then filled out on [REDACTED] stating that the employee went to the ER on her own time on [REDACTED] to get it looked at.
