What happened
While maintenance employees were repairing the DLI chain they used an overhead crane to hoist in the head shaft. The operator of the overhead crane moved the part to the left when the injured employee assumed the operator was moving the part down. The injured employee’s left hand was placed on the part to guide it into position when it came into contact with a support beam pinching the employee’s left middle finger in between the part and the support structure.
