Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the unsafe use of a personnel platform: a)Worksite, near loading dock - Two employees were being elevated by a rough terrain forklift truck (S/N 6036-0F1191) in a personnel platform. The following conditions were observed on or about 02/15/01: 1)The personnel platform was also lifting two galvanized sheet metal gutters, 24 feet in length. The two employees were crouched under a load being carried on top of the personnel platform guardrail system. This load was too large to accommodate both of the employees on the platform. 2)The platform was secured to the forklift with steel chains. The steel chains were too long for this application and allowed 7 to 8 inches of slack. This condition could allow the platform to shift or slip from its position while employees were elevated. 3)As the employees were being lowered by the operator, the boom and platform jerked up and down, bouncing the employees around. The operator did not ensure that the lifting mechanism was operating smoothly before lifting personnel. The operator did not lift and lower the boom slowly and with caution. 4)The truck was moving while the employees were being elevated. The truck was operating on the inclined driveway for the loading dock. The operator did not place the truck in neutral and set the parking brake while the employees were being elevated. ABATEMENT NOTE: Among other methods, one feasible method to reduce or eliminate this hazard is to: Follow national consensus standard ASME/ANSI B56.6 Safety Standard for Rough Terrain Forklift Trucks, Section 5.15, and the manufacturer's recommendations. ABATEMENT DOCUMENTATION AND VERIFICATION IS REQUIRED.
