Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by swinging molds and cores, molds and cores falling off the hoist due to damage to hoist pendant(s), illegible hoist pendant(s), and broken hook latches, and hooks using wire as latches, in that: (a)In the Core room, the Meyer II hoist pendant control buttons were not legible due to being covered with material, preventing an employee from being able to see which button controlled the trolley, bridge, up and down controls on the pendant for the hoist. (b)In the medium line weld booth #2, the pendant on the hoist was damaged in that holes were exposing the wiring and a cover was missing over one of the control buttons. (c)In side bay, the latch on the 1-ton Jet hoist hook was broken and replaced by wire being placed around the hook. (d)On the 1/2 ton Beebe Bros, wire was used around the hook as a latch. Among feasible abatement method to correct the hazards, ensure that when periodic and frequent inspections are conducted they include the hoist attachments such as the pendants for the hoists and hooks with latches and if damage is observed, the hoist and/or hook is removed from service until correction has be done to ensure that pendants and the hooks are free from recognized hazards as required by ASME B30.16-1993 for Overhead Hoists (underhung) and ASME B30.10-1993 for Hooks.-1993
