On the bridge site at Interstate 271 and route 322 in Mayfield Heights,
Ohio, two employees
securing girders with spud burs working off a pic with no fall protection
were exposed to a 21.5
foot fall.  A third employee was working off a 16.5 inch steel girder
above moving traffic with
no fall protection and was exposed to a 21.5 foot fall.  Feasible
abatement measures may include
but are not limited to nets, manlifts, beamwalkers and full-body harnesses
and lanyards.
