Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
Trailer 745, located at Groendyke Transport, Inc., 10201
Triangle Drive, Amarillo, Texas:  On or about July 21, 1992,
the employer did not protect the wash rack employees from the
recognized hazard of asphyxia, in that an employee was allowed
to enter a confined space (Chemical Transport Tank) without:
1.  Proper testing of the tanks atmosphere just prior to entry.
2.  Proper selection of respirators and other PPE based upon
the test results.
3.  Proper engineering controls to reduce gases/vapors to a
safe level.
4.  Proper rescue protocol.
Among other methods, a feasible and acceptable abatement
method to correct this hazard would be to ensure that:
1.  Proper testing of the tank's atmosphere just prior to entry.
2.  Make selection of respirators and other PPE based upon
the test results and review of the MSDS.
3.  Utilize proper engineering controls to reduce the
gases/vapors to a safe level (i.e. below the permissible
exposure level).
4.  Assure that in the event of a rescue attempt that employees
performing the rescue are not placed at risk.
