Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to inhalation hazards from possible leaks, ruptures or release
from systems, pipelines, vessels, and equipment handling liquid
chlorine:
The following condition conributed to the existence of these hazards:
(a)  Vaporizer platform on the 5th floor of #1 chlorine bleach plant
expansion chambers fabricated by the plant were not pressure
tested or pressure rated.  The employer has not implemented a
program for periodic inspection of the expansion chambers and
rupture disks.
NOTE:  Among other methods of feasible and acceptable methods
to correct this hazard include:
1)  Establishing rupture disk maintenance program.
2)  Follow ASME Boiler and Pressure Vessel Code, Section
VIII, Unified Pressure Vessels - 1968, with Addenda
for inpsecting pressure vessels.

The following condition contributed to the existence of these hazards:
(a)  Vaporizer platform on teh 5th floor of #1 chlorine bleach plant
where valves have been placed between the chlorine lines and
the expansion chambers.  This allowed the possibility that
the passages to the expansion chamber could be closed, thereby
defeating the purpose of the expansion chambers.
NOTE:  Among other methods, feasible and acceptable methods to correct
this hazard include removing the valves from between the
chlorine lines and the expansion chambers.
