death or serious physical harm to employees in that employees were
exposed to inhalation hazards from possible leaks, ruptures or release
from systems, pipelines, vessels, and equipment handling liquid
chlorine:
The following condition conributed to the existence of these hazards:
(a)  Vaporizer platform on the 5th floor of #1 chlorine bleach plant
expansion chambers fabricated by the plant were not pressure
tested or pressure rated.  The employer has not implemented a
program for periodic inspection of the expansion chambers and
rupture disks.
NOTE:  Among other methods of feasible and acceptable methods
to correct this hazard include:
1)  Establishing rupture disk maintenance program.
2)  Follow ASME Boiler and Pressure Vessel Code, Section
VIII, Unified Pressure Vessels - 1968, with Addenda
for inpsecting pressure vessels.

chlorine lines and the expansion chambers.
