Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
The hazards of fractures, internal injuries, lacerations,
concussion and electrocution fatality while using a Hi-Ranger
aerial boom platform mounted on a truck body while the turntable
controls were malfunctioning while elevated and rotated over
4 kv power lines.  The employee in the bucket platform was
exposed to the hazard of possible contact with the power lines
due to erratic control of the aerial boom turntable controls.
Among other methods, one or more feasible and acceptable
abatement methods to correct this hazard are, but are not limited
to:
1) Immediately take the unit out of service to repair the
erratic control operation.
2) Conduct periodic detailed inspections as specified in the
operating manual.
3) Keep and maintain a signed and dated record of each
inspection.
