The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to:
a.In the warehouse, on or about September 07, 2000, the employer
did not utilize written lockout procedures on Vertical
Transfer Lift # 85, thus exposing employees to a caught-in
hazard.  Among other methods, one feasible and acceptable
abatement method to correct this hazard is:
a.  Implement and utilize lockout procedures, i.e. disconnect
power source, attach lock and tag.
