29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: a. Located at the job site at 5189 Clark State Rd, Gahanna, Ohio: On or about May 21, 2024, and at times prior, a baker supported scaffold was in use onsite that was not fully planked or decked between the front uprights. There were bowed wooden 2" x 6" x 8' boards in use stretched across part of the scaffold, and onto the exterior of excavation with a piece of plywood in use on top of it. Employees were thereby exposed from falls to lower level ranging from approximately 2-5 feet from the platform of the scaffold while performing electrical installation work.

Located at the job site at 5189 Clark State Rd, Gahanna, Ohio: On or about May 21, 2024, and at times prior, a baker supported scaffold was in use onsite that was not supported by base plates, mud sills, or other adequate foundation. The scaffold was installed inside of an excavation with observed settling. As a result, employees were exposed to fall hazards when working from the baker supported scaffold to perform electrical installation work.

29 CFR 1926.451(c)(2)(iii): Unstable objects shall not be used as working platforms. a. Located at the job site at 5189 Clark State Rd, Gahanna, Ohio: On or about May 21, 2024, and at times prior, a baker supported scaffold was in use onsite that had unstable wooden boards in use as working platforms in that two wooden 2" x 6" x 8' planks were in use at an angle across the top of the baker support scaffold as well as a piece of plywood. The wooden boards were heavily bowed and unstable in the manner in which they were installed. As a result, employees were exposed to fall hazards ranging from approximately 2-5 feet from the scaffold while performing electrical installation work.

Located at the job site at 5189 Clark State Rd, Gahanna, Ohio: On or about May 21, 2024, and at times prior, a baker supported scaffold was in use onsite that was not visibly inspected by a competent person before each work shift. The scaffold was in unsafe condition in that the footing and uprights were not on base plates, mud sills, or adequate firm foundation; it was not fully planked; and unstable objects were used as the working platform. As a result, employees were exposed to fall hazards when working from the baker supported scaffold to perform electrical installation work.
