failure:
a)  Leitner-Poma of America, Inc. & 2510 Foresight Cr., Grand Junction, CO:
On or before 6/18/02, the employer did not ensure that cranes exhibiting
deficiencies
which could compromise their safe function were removed from service, as
required
by ASME B30.5-1994, The American Society of Mechanical Engineers - An
American National Standard - Safety Standard for Cableways, Cranes,
Derricks,
Hoists, Hooks, Jacks, and Slings - Mobile and Locomotive Cranes, paragraph
5-
2.3.2, until such deficiencies were repaired. The Grove RT60S (Serial#
19376) truck
crane exhibited deficiencies such as, but not limited to, intermittently
functioning
gauges and anti-two blocking device, a sticking boom angle indicator, and
cracked
welds on the outriggers.
Abatement Note: One feasible abatement method is to follow the
requirements of ASME
B30.5 and to remove the crane from service until the deficiencies are
corrected.

On or before 6/18/02, the employer did not ensure that shop-made
under-the-hook
lifting devices were tested and rated in accordance with the requirements
of ASME
B30.20-1985, The American Society of Mechanical Engineers - An American
National Standard - Safety Standard for Cableways, Cranes, Derricks,
Hoists, Hooks,
Jacks, and Slings - Below-the-Hook Lifting Devices. These deficiencies are
as
follows:
1.  Job-made lifting devices such as the hanger tube lifting clamp in the
Machine Shop and the sheave train lifting device in the Sheave Area were
not
marked as to their load rating, manufacturer, lifter weight, and serial
numbers
which are required by paragraphs 20-1.2.1 (a) and (b) of ASME B30.20-1985.
2.  The lifting devices had not had an initial rated load test, using the
attachment methods in place, as required by paragraph 20-1.4.2(a) of ASME
B30.20-1985.
Abatement Note: One feasible abatement method is to test and rate the
hoists and lifting
devices in accordance with ASME B30.20-1985.
