a.Arthur Ave Clarksburg, WV - The first truss that was installed in the
middle of the building and
working to the North, was not supported by temporary ground bracing to the
interior of the
building on or about 11-29-01.
b.Arthur Ave Clarksburg WV - The last truss placed on the North end of the
building was not
supported by temporary ground bracing to the exterior of the building on
or about 11-29-01.

a.Arthur Ave, Clarksburg, WV - (1)  There were only two continuous top
cord lateral braces on the
42 foot span scissor truss, starting at 31 inches up from the eave and
continue up the 271 inch long
cord to the peak, spacing  ten feet apart with a peak brace that
alternated from the east to the west
side of the peak (2) There was no lateral or diagonal bracing in place on
the bottom cord  of the
42 foot span scissor truss    (3) There was  a spacing of 15 trusses
between each top cord diagonal
brace in place on the 42 foot span scissor truss on or about 11-29-01 .

subject to bending and
breaking on or about 11-30-01.
