Cook Brothers Building & Excavating, Inc., Morgantown, WV:  On or about 10/07/15, the temporary receptacle box providing power to the Dewalt air compressors, Dewalt circular saws, and extension cords had ground fault circuit interrupters that were broken and not protecting employees.     Cook Brothers Building & Excavating, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.404(b)(1)(ii), which was contained in OSHA inspection number 594258, Citation 01, Item number 005 and was affirmed as final order on 02/07/13, with respect to a workplace located at South Pike Street, Shinnston, WV 26431.     ABATEMENT CERTIFICATION AND DOCUMENTATION IS REQUIRED.
