Section 5(a)(1): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a)On or about January 12, 2002, LOTO and/or alternative procedures were not utilized, during sanitation operations being performed on the Weiler final grinder located in the grinding department, to prevent outside contracted sanitation employee(s) from contacting the rotating mixer paddles. This condition exposed the employee(s) to amputation hazards. IN THE ALTERNATIVE 29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by rotating parts: (a)On or about January 12, 2002, adequate guarding was not provided on the Weiler grinder, located in the grinding department, in that outside contracted sanitation employees could reach into the mixer area of the grinder. This exposed the employee(s) to amputation hazards. No certification required. Violation Abated.
