(a) Location - West Tunnel, Framingham, MA:
On or about May 11, 2000 employees were injured in a collision between a
train
transporting concrete and a train transporting employees traveling on the
same track in
opposite directions.
Among other methods, one feasible means of abatement could include:
1. Develop and implement adequate written traffic control procedures for
trains
transporting employees and/or concrete inside the tunnel.
2. Ensure that all employees engaged in this operation receives adequate
training in these
written procedures.
