Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of electrical shock, burns or electrocution
while operating a drill
press which was in an unsafe operating condition. The following conditions
contributed to the
existence of these hazards:
(a)M/V U.S. Enterprise, engine room: The drill press, located on the
starboard side
of the vessel and to the left of the bench mounted grinder, was missing
its on/off
switch cover.  The switch was loose in its holder and the power cord was
not
properly attached to the junction box to prevent strain/pull on the
terminals/connection points (missing cable clamp).
Note: One feasible method of abatement, among others, is to tighten the
switch in place,
install a switch cover and to push the conductors slightly further into
the junction box and
tighten the cable clamp around them.

potential use of an ungrounded appliance in the presence of water exist.
The following conditions
contributed to the existence of these hazards:
(a)M/V U.S. Enterprise, forward factory deck bathroom:  Electrical
receptacles over
the sink were not protected by ground fault circuit interrupters (GFCI) at
either
the outlet or at the electrical panel.
(b)M/V U.S. Enterprise, upper crew deck head:  Electrical receptacles over
the sink
were not protected by ground fault circuit interrupters (GFCI) at either
the outlet
or at the electrical panel.
Note: One feasible means of abatement, among others, is to replace the
existing
receptacles with a GFCI.

were exposed to the hazard of electrical shock, burns or electrocution
while working in, on or
around an electrical circuit breaker panel which had a unused opening. The
following condition
contributed to the existence of the hazard:
(a)M/V U.S. Enterprise, engine room:  Unused openings in electrical
control panel
#3, MSB #111, revealed live electrical components and the employer failed
to
insure that the unused opening was guarded to prevent employees from
inadvertent contact with the live parts.
Note: One feasible means of method, among others, is to simply insert a
blank snap in
cover into the unused opening.
