Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of electrical shock, burn or electrocution from
unguarded light
fixtures and missing snap-ins on the electrical boxes:
The following conditions contributed to the existence of these hazards:
(a)F/T Ocean Peace:  In the change-out room, fluorescent light fixtures,
less than
8 feet from the floor, were without covers or protective covers on the
tubes.
(b)F/T Ocean Peace:  In the machine shop, fluorescent light fixtures less
than 8 feet
from the floor, were without covers or protective covers on the tubes.
(c)F/T Ocean Peace:  In the engine room, fluorescent light fixtures, less
than 8 feet
were without covers or protective covers on the tubes.
(d)F/T Ocean Peace:  In the laundry room, two fluorescent light fixtures,
less than
8 feet from the floor, had burned out lights and fluorescent tubes missing.
(e)F/T Ocean Peace:  In the machine shop, a drilling machine was missing
snap-ins
on the octagon electrical box and on the electrical switch.
(f)F/T Ocean Peace:  In the laundry room, the on/off switch box had two
snap-ins
missing.
NOTE:  One feasible means of abatement, among others, is to install/replace
covers and fluorescent tubes and replace snap-ins.

(a)F/T Ocean Peace:  In the galley, a receptacle next to the sink that was
not
protected with ground-fault circuit-interrupters (GFCI) at either the
outlet or the
electrical panel.
NOTE:  One feasible means of abatement, among others, is to install a
receptacle
or circuit breaker that is protected by a GFCI at each location.

electrical
parts exposed.
(b)F/T Ocean Peace:  In the galley, a GE pump, model #SKC38NG4289X, 3/4 hp,
3-phase, 115/230V, 3450 rpm had live electrical parts exposed.
NOTE:  One feasible means of abatement, among others, is to replace the
covers
on the hot water heaters and the GE pump.

where exposure to
energized equipment while performing any service or maintenance on a
machine or equipment
and the start up or release of stored energy could occur:
(a)F/T Ocean Peace:  The employer did not provide written documentation
for the
control of potentially hazardous energy when employees are engaged in these
activities.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard would be to develop and implement a written lockout/tagout
program
and conduct training to ensure that before any employee performs electrical
servicing or maintenance, the machine or equipment is isolated from the
energy
source and rendered inoperative.
