F/V Alaska Warrior, OSI Dock, Factor Shop; on or about 09 February 2018 and times prior thereto; the tongue guard on a six-inch Baldor Bench Grinder, Model 73060, Serial Number W0806142315 was missing. This condition exposed employees to a caught in hazard.

F/V Alaska Warrior, OSI Dock, Deck starboard adjacent to engine room entrance; on or about 09 February 2018 and times prior thereto; four oxygen cylinders and three acetylene cylinders were not separated by at least 20-feet or by a noncombustible barrier. This condition exposed employees to fire and explosion hazards.

29 CFR 1915.55(a)(1): Valve protection caps on gas Welding and cutting cylinders were not in place or secure.  F/V Alaska Warrior, OSI Dock, Deck starboard adjacent to engine room entrance; on or about 09 February 2018 and times prior thereto; three instances were noted where welding and cutting cylinders did not have the protective caps installed:       a. Deck starboard adjacent to engine room entrance; two oxygen cylinders were missing the protective caps.       b. Deck starboard adjacent to engine room entrance; one acetylene cylinder was missing its protective cap.  This condition exposed employees to fire and explosion hazards.
