29 CFR 1910.132(d)(1)(i):     The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:        F/T Seafisher, Galley:  On or about 16 February 2013 and times prior thereto, the cook did not have nor was required to use cut resistant gloves while using knives in the galley for food preparation.  This condition exposed the employee to laceration hazards.

29 CFR 1910.151(c):     Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    F/T Seafisher Factory:  On or about 16 February 2013 and times prior thereto,  Eye wash stations were not established while employees were using Super Red Plus.  This condition exposed employees to chemical hazards in the event of eye or face exposure to corrosives while cleaning the factory.

29 CFR 1915.89(c)(7)(ii):      The employer did not ensure the coordination process included a lockout/tags-plus coordinator and a lockout/tags-plus log.    F/T Seafisher Factory, OSI Dock:   On or about 16 February 2013 and times prior thereto, The lockout/tags-plus coordinator was not actively involved in the equipment de-energizing process and a lockout/tags-plus log was not established for Tokai headers locked out for routine servicing.   This condition exposed employees to a caught in and amputation hazard.
