Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to struck-by or crushing hazards:
a)Department 53 - On or about 11/30/10, employees were exposed to
struck-by and
crushing hazards when standing near or under a member being lifted.
b)Department 53 - On or about 11/30/10, employees were exposed to
struck-by and
crushing hazards when plate clamps did not receive complete maintenance
inspections
each month.
c)Department 53 - On or about 11/30/10, employees were exposed to
struck-by and
crushing hazards when damaged plate clamps were not taken out of service.
d)Department 53 - On or about 11/30/10, employees were exposed to
struck-by
and
crushing hazards when a large capacity clamp was used to lift a lighter
load that was
outside the design limitations.
e)Department 53 - On or about 12/2/10, employees were exposed to struck-by
and crushing
hazards when a vertical lifting plate clamp was used to lift loads
horizontally.
Among other methods, one feasible and acceptable method to correct this
hazard is to follow the
Renfroe Model LJ and FR Clamp Application, Operation and Maintenance
Manual and ASME
B30.20 Below-the-Hook Lifting Devices.
