North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to falling loads: When required, a latch was not provided to bridge the throat opening of the hook and retain, under slack conditions, such items as, but not limited to, slings and chains. (Reference ASME B30.16d-1.2.9-1997): a)tool room - two Coffing 1-Ton electric overhead (underhung) chain hoists with missing latch on hooks. b)injection molding - CM 1-Ton electric overhead (underhung) chain hoist missing safety hook latch on hook. c)tool room - Coffing 3-Ton electric overhead (underhung) chain hoist with sprung/bent latch. A feasible method of abatement is to ensure latches on hooks are properly installed and maintained.

North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to falling loads: Frequent inspections as defined in ASME B30.16-2.1.1(b)(1) and periodic inspections as defined in ASME B30.16-2.1.1(b)(2) were not performed in accordance with Tables 1 and 2 (Reference ASME B30.16-2.1.2 and 16-2.1.3): a)tool room - two Coffing 1-Ton electric overhead (underhung) chain hoists with missing latch on hooks and one Coffing 3-Ton electric overhead (underhung) chain hoist were not inspected. b)injection molding - CM 1-Ton electric overhead (underhung) chain hoist missing safety hook latch on hook was not inspected. c)tool room - Coffing 3-Ton electric overhead (underhung) chain hoist with sprung/bent latch was not inspected. A feasible method of abatement is to appoint a person to inspect and record external conditions of hooks throughout the facility in accordance with Tables 1 and 2 of ASME B30.16-1997 Section 16-2.2.
