employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to the hazard of being struck by
a falling load:
a) Silicon Coating Department, Hot Melt Coating Department, Polystar
Department - Custom roll
lifting devices which have been fabricated in house are used to lift rolls
of material weighing up to 800
pounds.  These home made lifting devices are constructed of steel rods
welded or bolted to a channel
steel spreader bar.  The steel bars are bent into a hook to accept the
roll shafts.  The devices attach to
the hoist hook by a center eye bolt, bolted or welded to the channel steel
spreader, or a steel rod bent
in a "U" shape and welded to the channel steel spreader.  These devices
were not proof tested or
provided with a rated load capacity attached to the lifting device.
ABATEMENT NOTE:  One feasible and acceptable method to abate this hazard
is
to provide a lifting device
designed and manufactured in accordance with ANSI B30.20, Below-the Hook
Lifting Devices, latest edition.
Abatement certification required within 10 days after abatement date.  The
certification shall include a
statement that abatement is complete, the date and method of abatement,
and state that employees and
their representatives were informed of this abatement.tives
