Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard due to failure of below the hook lifting devices: Welding Area - Below the hook lifting devices were used throughout the area and numerous deficiencies were noted such as; all devices were made in-house with no documentation of the lifting capacity, no proof testing for rated load, and components not used properly as noted; (a) Welding Area - PC 300 Hood lifting device consisted of non-alloy chain links with metal L-plates. One of the anchor shackles was mounted the opposite way onto plate, no packing (washers) to centralize the load. (b) B-5 Assembly Area - Below the hook lifting device used to lift Komatsu door had shoulderless shackle mounted at an angle (designed to be mounted vertical) and the wire rope sling had a clip on backwards. (c) Welding Area- PC Hood lifting device consisted of 2 wire rope legs with lifting plates. The wire rope was attached to metal plates by forming an eye, there was no thimble in eye to prevent crushing of rope. Among other methods, one feasible and acceptable abatement method to correct this hazard is for the employer to follow manufacturer guidelines for lifting components and use the ANSI B30.20 Below- the-Hook-Lifting Devices as a reference.
