potential into proximity to parts which were not disconnected from a
source of electrical energy.
a)An employee performing switching operations at the 1531 North Water
Street
Pfister & Vogel tannery site intiated an arc blast by bringing an object
energized at a
different potential into proximity with electrical parts energized at
13,200 volts phase-to-
phase and 7620 volts phase-to-ground.  All potential sources of electrical
energy had not
been opened/de-energized, tested and grounded.
Among other methods, one feasible and useful method of abating this hazard
is to ensure
that all affected electrical parts are disconnected from their source(s)
of energy, tested
and determined to be de-energized, and visibly grounded before being
approached with
objects which are at ground potential, or energized at a different
potential above ground.
