General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious harm to employees in that employees were potentially exposed to amputation hazards and crush injuries from unintended/accidental machine activation: a) HTC Hydraulic Shear - The foot pedal for the shear was not guarded to prevent unintentional/accidental machine activation. ANSI B11.4 Shears - Safety Requirements for the Construction, Care and Use. Section 4.3.1.1 requires foot-operated tripping controls to be protected by a guard to prevent accidental actuation by falling or moving objects or someone accidentally stepping them. ABATEMENT NOTE: Among other methods, one feasible and acceptable method to correct this hazard is to guard the foot pedal with a guard in accordance with ANSI B11.4, Section 4.3.1.1.
