29 CFR 1910.134(c): The employer did not develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use:     a) Industrial Equipment Fabricators, Inc., 2 Early St. in Ellwood City, PA.  On or about August 15, 2013, a written respiratory protection program was not developed.  Personnel were using 3M 5000 Series Respirators with #6001 Organic Vapor Filter Cartridges.  The employer required use of the respirators during spray painting operations of the Sherwin-Williams KEM BOND HS High Solids Alkyd Universal Metal Primer.

On or about August 15, 2013, employees were not medically evaluated to determine if they could wear the 3M 5000 Series Respirators with #6001 Organic Vapor Filter Cartridges.
