B.) Industrial Equipment Fabricators, (Mubea Iron Worker Model KBL Plate Shear Section of Machine) On or about 7/18/24 and times prior thereto: Shear points and moving parts were not protected on the drop-off side by a guard or device. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Industrial Equipment Fabricators, On or about 7/18/24 and times prior thereto: Equipment including but not limited to the, Mubea Iron Worker Model KBL Coper-Notcher was not adequately guarded to prevent employees from contacting the point of operation in that the machine was operated with the guard in the retracted position. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).
