Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to electric shock or electrocution while in a confined space: (a)Union Tank Car Company, Inc., 16923 Old Beaumont Hwy 90, Houston, TX, 77049, Department 663, employee working inside a steel tank with portable light plugged into extension cord powered from a Miller Dimensions Model 652 Arc Welder Asset #16066 and no ground fault circuit interrupters (GFCI) protection. One feasible and useful abatement method to correct this hazard includes, but is not limited too, providing GFCI protection for employees utilizing hand tools and equipment. *** Certification and Documentation of Abatement Required ***

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential fire and/or dropped loads: (a)Union Tank Car Company, Inc., 16923 Old Beaumont Highway 90, Houston, TX, 77049, 10-Ton #2 Stacker Crane, magnetic lift assembly, an under-the-hook lifting device, UTC #17510, DC power conduits separating from boxes and rags stuffed into DC electrical box around conductors. One feasible and acceptable method of abatement to correct this hazard includes, but is not limited to, conduct frequent inspections of electrical conductors on lifting magnets and correct defective conditions disclosed by the frequent inspections as outlined in ANSI Standard B30.20 Under-The-Hook Lifting Devices, Section 20-3.3 - Inspection, Testing, and Maintenance. *** Certification and Documentation of Abatement Required ***
