Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being caught in rotating parts of an
electric pipe threader:
a) Warehouse area. On or about 6/27/01, the electric pipe threader was
activated with
a manually operated REVERSE/OFF/FORWARD switch which would not
automatically
cause the machine to shut off in the event an employee got caught in
rotating parts.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to install a constant pressure control switch as required by the
American
National Standards Institute B208.1 Section 4.4.
