Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by pipes should they become
unsecured, or by falling
loads if the hoists or cables or lifting devices fail:
a)Bennett Property, On or about 7/6/01:  Return Mud Line was not secured so
that it would not come loose.
1.Return Mud Line, Rigid return line was not secured to prevent
displacement.  The return line did come loose, causing serious injury.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to:
1.Follow guidelines as referenced in API 54, Section 9.13.3, 1999,
Recommended Practice for Occupational Safety for Oil and Gas Well Drilling
and Servicing Operations, by securing lines.
b)Kienzle Property #1, On or about 7/20/01:  Below the hook lifting
devices had
no rated load markings and no initial, frequent or periodic inspections
performed, including, but not limited to:
1.On the rig floor, a spreader type lifting device had no rated load
markings and no inspections were conducted.
Among other methods, several feasible and acceptable methods to correct
these
hazards are to:
1.Inspect prior to initial use, all new, modified or repaired lifting
devices by a
designated person and perform an operation test.
2.Mark the rated load capacity of the lifting device on the main structure
or
equivalent.
3.Conduct frequent and periodic inspections of the lifting devices for
wear,
deterioration or malfunction as referenced in ASME B30.20.
4. Follow guidelines as referenced in API 54, Section 9.7.1, 1999,
Recommended
Practice for Occupational Safety for Oil and Gas Well Drilling and
Servicing
Operations.
c)Kienzle Property #1, On or about 7/20/01:  Hoists, Derricks, and Hoisting
lines were not inspected such as, but not limited to:
1.Dozer mounted hoists were not provided with load ratings and
inspections.
2.Dozer mounted booms were not provided with load ratings and
inspections.
3.Cable connections were not rated and were not provided with
inspections.
Among other methods, several feasible and acceptable abatement methods to
correct
these hazards are to:
1.Mark the rated loads on the hoists and structures.
2.Test all new, repaired, or modified equipment prior to use.
3.Provide proper hook to cable connections instead of knots.
4.Follow guidelines as referenced in API 54, 1999, Recommended Practice for
Occupational Safety for Oil and Gas Well Drilling and Servicing Operations.
5.Follow guidelines as referenced in ANSI B30.7, Base Mounted Drum Hoists.
Abatement certification must be submitted for this item.

a)Kienzle Property #1, On or about 7/20/01:  Auxiliary means of egress
(such
as Geronimo Lines) were not provided from the Derrickman's working
platform.
Among other methods, several feasible and acceptable methods to correct
these
hazards are to:
1.Provide a secondary means of escape (such as a Geronimo Line) for
employees required to work on the Derrick Platform.
2.Follow the requirements of API 54, Section 6.10.1, 1999.
