were exposed to the hazard of being struck by falling loads if the hoists
or cables or lifting
devices fail:
a)Roy #1, On or about 7/8/01:  Hoists, Derricks, and Hoisting lines were
not
inspected such as, but not limited to:
1.Dozer mounted hoists were not provided with load ratings and
inspections.
2.Dozer mounted booms were not provided with load ratings and
inspections.
3.Cable connections were not rated and were not provided with
inspections.
Among other methods, several feasible and acceptable abatement methods to
correct
these hazards are to:
1.Mark the rated loads on the hoists and structures.
2.Test all new, repaired, or modified equipment prior to use.
3.Provide proper hook to cable connections instead of knots.
4.Follow guidelines as referenced in API 54, 1999, Recommended Practice for
Occupational Safety for Oil and Gas Well Drilling and Servicing Operations.
5.Follow guidelines as referenced in ANSI B30.7, Base Mounted Drum Hoists.
Abatement certification must be submitted for this item.
