Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard being entangled in the rotating roto-mill
cutting drum:
(a) Hamon Contractor's Inc. -  Stapleton Airport Project: At approximately
8:50pm, an employee was observed changing the bits on the cutting drum with
a pneumatic wrench without shutting off the engine to the Roadtec RX-60B
roto-
milling machine.  The employee would have sustained serious injuries, if
the
drum were inadvertently activated.  The company protocol for isolating the
drum
from activation was not an approved method recommended by the
manufacturer.
Abatement Method: The bits on the cutting drum can be safely changed by
following the safety procedures set out by the manufacturer in Sections
1.4 titled
"Machine Safety Precautions" and in 4.3 titled "Maintenance" of the
Roadtec RX-
60B Operations and Service Manual.
Abatement Note:  Abatement Certification and documentation are required for
this item (see enclosed "Sample Abatement-Certification Letter").
