Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
Employees working with, and in the vicinity of, cranes and hoists in the
shop and yard areas
were exposed to the hazard of being struck by falling loads in that the
cranes and hoists were
not inspected and tested on a frequent (daily to monthly) basis, and
cranes and hoists were not
maintained in a safe operating condition. Cranes and hoists remained in
service with unsafe
conditions including unlabeled or illegibly labeled pendant control
functions, missing load hook
safety latches, pendant control cables pulled loose from the control box,
worn / damaged wire
rope hoist cables, worn or out of adjustment hoist load holding brakes,
and motor conductors
with the outer cord insulation pulled loose from the cord grip exposing
inner
conductors.
Among others, one feasible and acceptable method to correct this hazard is
to implement a crane
and hoist inspection program that complies with ANSI B30.11 the American
National Standard
for Monorail Systems and Underhung Cranes, and / or ANSI B30.16, the
American National
Standard for Underhung Hoists, and to repair or remove from service cranes
and hoists with
defects noted during the inspection.
