were exposed to the hazard of amputation:
(a)Bay 2; Pacific Press Brake #2: On April 2, 2003, two persons were
required to operate
the Pacific Mechanical Press Brake #2 to bend or form a large piece of
metal.  The press
brake was operated by one foot control (pedal) which exposed employees to
the hazard
of hand/finger amputations.
Among others, one feasible and acceptable means of abatement would be to
follow the
recommendations of the American National Standards Institute, ANSI
B11.3-1982, Safety
Requirements for Construction, Care and Use of Power Press Brakes, Section
E6.1.4.,
Safe Guarding the Point of Operation.  Paragraph E6.1.4.2.6 (3),

Controls, states that, "In a piece-part bending operation requiring more
than one operator
(helper), separate hostage controls shall be provided for each (operator)
helper."
