Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish to each of his employees a place of employment which was free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees.  In that
employees were exposed to the recognized hazard of being struck or crushed
by the hoisted load
and the crane body/boom of the 80 ton Link Belt Lattice Boom Crawler Crane
Model Number
LS - 138 II Serial Number H7J1-5924 when this crane was operated by an
operator that had
received training and/or examinations on a pedistal crane.
a)  Among other methods, one feasible method and acceptable abatement to
correct this
hazard is to ensure that operator examinations are limited to the specific
type equipment
they will operate, in accordance with section 5-3.1.2a of the American
National
Standards Institute (ANSI) B30.5-1968, Crawler, Locomotive and Truck
Cranes.

LS - 138 II Serial Number H7J1-5924.  The crane operator initially
informed
the drilling
company Tool Pusher that he felt it was unsafe to travel the crane with
the load suspended.  Yet
he continued to travel the crane as directed by the tool pusher. This
resulted in the collapse of
the boom.
a)  Among other methods, one method of abatement is to ensure that the
crane operator
shall be responsible for those operations under his direct control.
Whenever there is any
doubt as to safety, the operator shall have the authority to stop and
refuse to handle loads
until safety has been assured, in accordance with section 5-3.1.3d of the
American
National Standards Institute (ANSI) B30.5-1968, Crawler, Locomotive and
Truck
Cranes.

act as the signal man
for the operation.

5-3.1.3(c) of the American National Standards Institute (ANSI) B30.5-1968,

hazard is to ensure that firm footing, uniformly level within one percent
is provided.
Where such footing is not otherwise supplied, it should be provided by
substantial
timbers, cribbing, or other structural members sufficient to distribute
the load so as not
to exceed the safe bearing capacity of the underlying material in
accordance with section
5-3.4.6 of the American National Standards Institute (ANSI) B30.5-1968,
Crawler,
Locomotive and Truck Cranes.
