Section 5(a)(1): Each employer shall furnish to each of his employees
employment and a
place of employment which are free from recognized hazards that are
causing or likely to
cause death or serious physical harm to his employees.
On Rig #391, wire rope clips used to hold the blow out preventer did not
use the appropriate
number of clips and the U-bolts were put on wrong (the U-bolts were on the
live line). Also,
the eye did not have a thimble. One wire rope on the drill floor was not
turned back properly
and the clips were not evenly spaced. The wire rope on one snub line was
bird caging out of
the thimble. One of the air hoses on the air hoist was split and needed to
be replaced. The
pipe rack did not have chocks or wedges to secure the pipe from rolling. A
stairway by the
pipe rack was damaged and not taken out of service or repaired. The side
doors on the mud
house was not properly secured to protect employees from a fall hazard.
Feasible and acceptable methods to correct these hazards are:
1. Use proper rigging techniques for use of wire rope clips as outlined in
1926.251 Table H-
20. Inspect the wire rope terminations on a daily basis.
2. Replace the split air hose line.
3. Use chocks or other devices to secure pipe joints on the pipe rack.
4. Secure the side doors on the mud house.
