Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were causing or likely
to cause death or serious physical harm to employees in that employees
were exposed to:
The hazard of possible hydrogen cyanide gas generation.  In the Chemical
Storage Room, there were
stored containers of cyanide salts and drums of sulfuric, hydrochloric,
phosphoric, and nitric acid.  In
a disruptive event, where the containers are broken and there is spillage,
the cyanide salts and the acid
solutions could intermix and generate hydrogen cyanide gas.
A feasible control to remedy this hazard is the separate storage of the
cyanide salts in a diked area or
other area outside of the drainage of the Chemical Storage Room.
