29 CFR 1910.132(e): The employer did not remove from service defective or damaged personal protective equipment. On the chemical film line at 68 North Elm Street Westfield, MA: On or about February 18, 2016, the employer did not ensure that a pair of Neoprene gloves were not damaged before the chemical film line supervisor used them while performing a small batch etching process with a Nitric and Hydrofluoric acid solution which resulted in skin contact with the acid on the supervisor's right hand thumb. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
