Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY MOBILE EQUIPMENT: a)Facility Wide, on or about 6/12/03: A New Holland model LW130 front end loader, serial number 2EFOLW13000551107 being operated by employees to move cellulosic material had a reverse signal alarm that was not functioning at all times. ABATEMENT NOTE: One means of abatement is to repair the reverse signal alarm and follow the operator's manual and the safety manual(Association of Equipment Manufacturers) by having employees check to ensure the reverse signal alarm is functioning properly.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING CRUSHED-BY MOBILE EQUIPMENT: a)Facility Wide, on or about 6/19/03: The Mitsubishi model FG-40 fork lift, used by employees for miscellaneous lifting was not equipped with a seat belt. ABATEMENT NOTE: One means of abatement is to re-install a seat belt which was part of the original equipment on this machine and ensure that employees operating this fork lift wear the seat belt at all times during operation.
