29 CFR 1910.145(f)(5): Danger tags were not used where an immediate hazard presented a threat of death or serious injury to employees:  a) Steel mezzanine, on or about 6/27/13: Danger tags or warning signs were not posted warning employees to stay away from area under mezzanine because of structural instability. The area was not cordoned off.  b) Loading dock area, on or about 6/27/13: Danger tags or warning signs were not posted warning employees to stay away from loading dock walls due to structural instability and vibration from tractor trailer use. The area was not cordoned off.  c) Hallway from break room to maintenance shop, on or about 6/27/13: Danger tags or warning signs were not posted warning employees not to enter hallway because of unstable steel push wall. The area was not cordoned off.  Abatement cerification must be submitted for this item.
