Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY THE LOAD or WATER JET SPRAY: a)Water Jet Area, on or about 1/8/04: Two IRB 3000/3200 Abrasive Water Jet systems were not adequately safeguarded in that the sensitive mats provided were not functioning. Among other methods, on feasible method of abatement is to conduct routine inspection of machine safeguards to ensure their effectiveness. b)Etching Area, on or about 1/9/04: Two below the hook lifting devices had unknown load capacity ratings and were not being inspected on a frequent and periodic basis. Among other methods, one feasible method of abatement is to conduct frequent and periodic inspections of the lifting device and mark the lifting device with the rated load capacity as per ASME B30.20-1993. Abatement certification must be submitted for this item. c)Throughout the facility, on or about 1/9/04: The following hoists had defective or were missing safety latches from the load hook: 1)Coffing 1/2 ton hoist, located in the Etching area. 2)Coffing 1/2 ton hoist, located in the Beacon Line. 3)P & H crane, located in Heat Treat. Among other methods one feasible and acceptable method to correct this hazard is to replace the missing and/or defective safety clips and to perform frequent and periodic inspections of the lifting devices as per ASME B30.16-1993.
