29 CFR  1910.147(c)(4)(ii):Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  On or about July 16, 2024, the machine specific procedures provided by the employer for the Amada Band Saws in the Raw Materials process of the Saw Department did not include a scope, purpose, authorization, rules, a specific statement of the intended use of the procedure, specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy, specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them, or specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. The procedures provided rely on an interlock and machine power switch, with no applications of locks, in lieu of a full lock out procedure. Employees are exposed to potentially hazardous energy such as, but not limited to, electricity, hydraulics, and mechanical sources when they change the blade on the saws.

Employees in the Raw Materials process of the Saw Department are exposed to energy sources including, but not limited to, electricity, hydraulics, and mechanical when they perform maintenance activities including, but not limited to changing the inserts and jaws on the CNC Lathe number 47659 and the blade on the Amada Band Saws.

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a. On or about July 16, 2024, employees who conducted servicing and maintenance activities including, but not limited to, changing the the inserts and jaws on the CNC Lathe number 47659 were exposed to electrical, hydraulic, and mechanical energy sources. Although the interlocked door was opened up to perform the work, the remaining applicable energy control elements involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from caught in hazards.   b. On or about July 16, 2024, employees who conducted servicing and maintenance activities including, but not limited to, changing the blade on the Amada band saws in the Saw Department were exposed to electrical, hydraulic, and mechanical energy sources. Although the e-stop was hit and an interlocked arm was opened up to perform the work, the remaining applicable energy control elements involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from caught in hazards.

29 CFR  1910.147(c)(6)(i):The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:  On or about July 16, 2024, the employer did not conduct an annual inspection of the energy control procedures to ensure that the procedure and requirements of this standard were followed.  Employees in the Raw Materials process of the Saw Department are exposed to energy sources including, but not limited to, electricity, hydraulics, and mechanical when they perform maintenance activities including, but not limited to changing the inserts and jaws on the CNC Lathe number 47659 and the blade on the Amada Band Saws.  No audit had been performed in 2024 prior to the initiation of the inspection and certification for the 2023 lock out audits were not available from the company.  Turbine Engine Component Technologies-Utica Corporation dba Whitcraft was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(6)(i), which was contained in OSHA inspection number 1604499, citation number 1, item number 3 and was affirmed as a final order on December 27, 2022, with respect to a workplace located at 8273 Halsey Road, Whitesboro, NY 13492.
