were causing or likely to cause death or serious physical harm to

were exposed to THE HAZARD OF BEING STRUCK-BY THE LOAD:
a)In the production area, on or about 06/18/04:  Below-the-hook lifting
devices
had no rated load markings and no initial, frequent or periodic inspections
performed.
Among other methods, several feasible and acceptable abatement methods to
correct
these hazards are to:
1.Inspect prior to initial use, all new, modified or repaired lifting
devices by a
designated person and perform an operational test to verify compliance.
2.Mark the rated load capacity of the below-the-hook lifting device on the
main
structure or equivalent as referenced in the American Society of Mechanical
Engineers (ASME) document B30.20-1993, section 20-1.2.1.
3.Conduct frequent and periodic inspections of the lifting devices for
wear,
deterioration or malfunction as referenced in American Society of
Mechanical
Engineers (ASME) document B30.20-1993, section 20-1.3.1.
b)In the production area, on or about 06/18/04:  Several overhead hoists
had
hook safety latches which were broken or missing.
Among other methods, several feasible and acceptable abatement methods to
correct
these hazards are to:
1.Replace broken safety latches on the hooks as referenced in the American
National Standards Institute (ANSI) document B30.16-1973, section 16-
2.1.2.10.
2.Install safety latches on hooks which do not have them referenced in the
American National Standards Institute (ANSI) document B30.16-1973, section
16-2.1.2.10.
Abatement certification must be submitted for these items.
