were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to THE HAZARD OF BEING STRUCK-BY:
a)In the production area, on or about 4/3/2007: Below-the-hook lifting
devices,
including but not limited to the basement fixed central lift and the mix
room barrel
lifter, had no rated load markings and no initial, frequent or periodic
inspections
performed.
Among other methods, several feasible and acceptable abatement methods to
correct
these hazards are to:
1.Inspect the lifting devices in accordance with American Society of
Mechanical
Engineers (ASME)  B30.20-1993 Section 20-1.3.
2.Mark the rated load capacity of the below-the-hook lifting device on the
main
structure or equivalent as referenced in the ASME document B30.20-1993,
section 20-1.2.1.
b)In the production area, on or about 4/3/2007: Overhead hoist for mixing
vessels had a missing safety latch.
Among other methods, several feasible and acceptable abatement methods to
correct
these hazards are to:
1.Replace broken safety latch on the hook as referenced in the American
National Standards Institute (ANSI) document B30.16-1993, section 16-1.2.9.
2.Install safety latch on the hook which does not have it as referenced in
the
American National Standards Institute (ANSI) document B30.16-1993, section
16-1.2.9.
Abatement certification must be submitted for this item.
