a)On or about 10/21/04-12/01/04, unsafe conditions, including load chain
twisted and excessively worn, and spreading of the lower block hook over
10, disclosed during frequent or periodic inspections of a 1 ton P&H
Zip II hoist, serial #HO2335 were not corrected by repair or replacement
before operation of the crane resumed.  Feasible and acceptable methods
of abatement include, but are not limited to, correction by repair or
replacement of hazardous conditions disclosed by frequent or periodic
inspections of the hoist prior to resumption of operation of the hoist,
as described in ASME B30.16-1998, 16-2.3.3.
