Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following conditions: a)On or about 10/21/04-12/01/04, unsafe conditions, including load chain twisted and excessively worn, and spreading of the lower block hook over 10, disclosed during frequent or periodic inspections of a 1 ton P&H Zip II hoist, serial #HO2335 were not corrected by repair or replacement before operation of the crane resumed. Feasible and acceptable methods of abatement include, but are not limited to, correction by repair or replacement of hazardous conditions disclosed by frequent or periodic inspections of the hoist prior to resumption of operation of the hoist, as described in ASME B30.16-1998, 16-2.3.3. *** Certification and Documentation of Abatement Required ***
