Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
fall hazards while being lifted on a
forklift truck without an approved personnel lift platforms or any form of
fall protection.
At this residential roofing construction job site:  Employees were hoisted
to the roof work area while standing
on the forks of a powered industrial truck.  Employees were not protected
from falls by any form of fall
protection or safety platforms.
While ultimate responsibility for correcting the hazard rests with the
employer, given his/her superior
knowledge of the work place, feasible and acceptable methods to correct
this hazard among others are:
1.  Follow guidelines listed in the American National Standards Institute
(ANSI) B56.1, Safety Standard for
Powered Industrial Trucks, section 4.17 - Elevating Personnel - with a
High
Lift Truck designed to elevate
personnel and provide the restraining means such as railing, chains,
cable, body belts(s) with lanyard(s), or
decelerating devices, that are in place and properly used.
Abatement certification is required for this violation.  The abatement
certification sheet is enclosed with the
citations.ion
