were exposed to struck by and crushing hazards from a mobile crane being
used:
On or about June 10, 2009, employees were exposed to struck-by and
crushing hazards in that
the employer did not ensure that the employees that were operating and
driving the Grove TM-
875, 80 ton mobile crane were properly trained on the operational
characteristics, controls, and
emergency control skills, such as response to fire, power line contact,
loss of stability, or control
malfunction. The crane operator and driver did not recognize and take the
necessary precautions
to ensure the safe operation of the crane.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO
CORRECT THIS HAZARD IS: To comply with the American Society of Mechanical
Engineers,
ASME B30.5 2007 Safety Standard for Mobile and Locomotive Cranes, Section
5-3.1.2.
Qualifications for Operators: (b) Operator requirements shall include, but
not be limited to, the
following: (1) evidence of successfully passing a physical examination.
(2) Satisfactory
completion of a written examination. (3) Demonstrated ability to read,
write, comprehend, and
use arithmetic and a load/capacity chart. (4) Satisfactory completion of a
combination written
and verbal test on load/capacity chart usage that covers a selection of
the configuration. (5)
Satisfactory completion of an operation test. (6) Demonstrated
understanding of the applicable
sections of the B30 Standard and federal, state, and local requirements.
Certification of
abatement is required by 29 CFR Part 1903 and is to be submitted within
ten (10) calendar days
after the abatement date.

the crane lifting device was not rated prior to initial use. The
below-the-hook lifting device
(Spreader Beam Assembly) used to rig the 17,000 lbs. to 32,000 lbs.
concrete panels, did not
have the rated load visibly marked on it or identification affixed listing
the manufacturers name
and address, serial number and lifting device weight. This exposed the
crane operator, rigger
and other employees assisting in the lift to a struck by and crushing
hazard, in that the lifting
capacity of the lifting device is unknown and can be exceeded, causing
failure to occur and the
load being dropped.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO
CORRECT THIS HAZARD IS: To comply with the American Society of Mechanical
Engineers,
ASME B30.20-2006 Below-The-Hook Lifting Devices, section 20-1.2.1 Marking,
which requires
the marking of the rated load capacity on all lifting devices legibly
marked on the main structure
or on a tag attached to it where it is visible. Certification of abatement
is required by 29 CFR
Part 1903 and is to be submitted within ten (10) calendar days after the
abatement date.
