SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. Employer knew or should have known that employee elevated to a height in excess of 20 feet above ground level while on wooden platform, without upper controls, mounted to forks of rough terrain forklift, without operator at the forklift's controls, at south end of Building 46, on or about March 12, 2004, was exposed to the hazard of not being able to control the forklift in an emergency situation. A feasible and useful method to correct this hazard, among other methods, is to have the operator remain at the controls of the forklift at all times the employee is on the platform. B. Employer knew or should have known that employees installing metal plate connected wood trusses at Building 45 were exposed to the hazard of the trusses toppling and falling to the third floor due to lateral and diagonal braces of insufficient length, numbers, and placement being used as temporary bracing. A feasible and useful method to correct this hazard, among other methods, is to obtain and follow the instructions and recommendations in the HIB-91 Summary Sheet- commentary and recommendations for handling, installing and bracing metal plate connected wood trusses or equivalent. C. Employer knew or should have known that employees installing metal plate connected wood trusses at Building 46 were exposed to the hazard of the trusses toppling and falling to the third floor due to lateral and diagonal braces of insufficient length, numbers, and placement being used as temporary bracing. A feasible and useful method to correct this hazard, among other methods, is to obtain and follow the instructions and recommendations in the HIB-91 Summary Sheet - commentary and recommendations for handling, installing and bracing metal plate connected wood trusses or equivalent.
